1. How do I obtain an insurance card for my dental?

Dental cards are not issued. You don’t need an ID card for dental. When visiting a Delta Dental Premier or Delta Dental PPO dentist, simply provide your social security or identification number. The dental office can use that information to verify your eligibility and benefits.
If you still would like an ID card, you can print a customized ID card on demand. Log in to www.deltadentalins.com “Online Services,” click the "Eligibility & Benefits" tab to view your eligibility and benefits information and to print an ID card. If you haven’t registered for Online Services, click on "Register Today" for an easy three-step registration process.

2. How do I find an in-network dentist?

Visit www.deltadentalins.com
Look to the right side of the home page to locate "Find A Dentist"
Enter your preferred zip code
Select "Premier" for the network
This search will return PPO and Premier providers, both of which are in the network.
- PPO providers offer the best value / most competitive rates
- Premier providers are also in the network

3. How do I find out if my dentist is in the network?

Enter your dentist's name into the search tool at www.deltadentalins.com.

See if your dentist is listed
