To Get Reimbursed for a Prescription:
Submit a Prescription Drug Claim Form to Navitus.Costco Rx Mail Service
If you would like to register for mail service, you can:
- Review the Costco Mail Order Brochure
- Submit the Costco Mail Order Registration Form
- Contact Costco Mail Order at 1-800-607-6861 with any questions (available M-F from 5:00 am - 7:00 pm and Saturday from 9:30 am - 2:00 pm
- Have your physician fax a prescription to Costco at 800-633-0334.
At the pharmacy and don't have your insurance card handy? This information may be helpful:

Call Anthem's Member Services at 800-825-5541 to get a new card mailed to you or for help printing out a Temporary ID Card.
