Medical Benefits FAQ
 
1. How do I download a summary of my benefits?
Click on your plan to download a Summary of Benefits.
 
2. How do I get a new insurance card?
  • Call Member Services at 1-800-825-5541 to request that a new insurance card be mailed.  If it is time sensitive, they can also show you how to print out a temporary insurance card.

3. How do I add a dependent child?  
  • If outside of Open Enrollment, you must provide proof of a Qualifying Event* within the last 30 days (*see question #4 for a definition of Qualifying Event).
  • Download and complete the "dependent changes" section on the Membership Change Form
  • Provide a copy of your child's birth certificate (a keepsake copy issued from the hospital is okay, so long as the parent's full name is on it)
  • Return to Risk Manager
  • The coverage will be effective on the first of the month in the month after the Qualifying Event took place (for example: if you had a Qualifying Event on July 17th, the coverage would begin on August 1st)
 
4. How do I add a dependent spouse?
  • If outside of Open Enrollment, you must provide proof of a Qualifying Event* within the last 30 days (*see question #4 for a definition of Qualifying Event).
  • Download and complete the "dependent changes" section on the Membership Change Form
  • Provide a copy of your marriage certificate 
  • Provide a copy of the front sheet of your most recently filed taxes (SISC uses this as another form of proof of marriage; the only time this isn't required is if you are newly married and tax season has not occurred since you've been married)
  • Return to Risk manager
  • The coverage will be effective on the first of the month in the month after the Qualifying Event took place (for example: if you had a Qualifying Event on July 17th, the coverage would begin on August 1st).
 
5. How do I terminate a dependent child or spouse?
  • Download and complete the "dependent changes" section Membership Change Form
  • Provide proof of a Qualifying Event within the past 30 days
  • Return to Risk Manager
  • The coverage will be canceled on the first of the month in the month after the Qualifying Event took place (for example: if you had a Qualifying Event on July 17th, the coverage would be terminated on August 1st).
 
6. What is a Qualifying Event?
  • SISC requires proof of a Qualifying Event within 30 days to add or drop dependents outside of Open Enrollment.  Some common Qualifying Events include loss of other coverage, eligibility for other new coverage, death, divorce, birth, adoption, etc.  Letters showing enrollment in or loss of new coverage must show the effective date.
 
7. How do I file a claim to get reimbursed from Anthem?
  • Download and complete  Member Claim Form  mail it to Anthem
  • If you have questions or need assistance, call Member Services at 1-800-825-5541
 
8. How do I change my name?  Or my dependent's name?
 
9.  How do I change my address?  
  • Login to your employee portal to make changes to your addresss: Employee Portal
 
10. I heard I am entitled to a medical second opinion.  Is that true?

11. How do I find an Anthem PPO medical provider (in network)?
